Youth Consent Form: If a teen is a minor their parent or their legal guardian must complete this consent form. If this form is not completed and signed the participant will not be permitted to engage in any activities conducted by Group Dynamix that require harnesses or any other activities where permission and acknowledgement of risk would reasonably be sought.
